Eligibility and Selection Criteria for PA Students who will begin clinical rotations in 2009.

A. Residency Requirement
Applicant must provide documentation of West Virginia residency or documentation proving current attendance in a West Virginia Physician Assistant Program.

B. Physician Assistant Program Requirement
Applicant must provide documentation that final clinical year will be entered in the same calendar year the scholarship award is made.

C. Scholastic Requirement
Applicant must provide program documentation of scholastic achievement either in form of cumulative grade point average (GPA) or percentile score.

D. Student Academy Involvement
Applicant must provide copies of membership card or other documentation of involvement in PA organizations:
1. Membership in the local, state or national student societies of the Academy of Physician Assistants (local school chapter, WVAPA, SAAPA, etc.).
2. Documents stating offices held, committee assignments, etc., in PA student organizations including dates and particulars.

E. Financial Requirement
Applicant must provide documentation of:
1. Financial need as defined by being eligible for federally financed financial aid. Documentation must be signed by financial aid officer.
2. Applicant must submit a signed statement acknowledging their responsibility to report the award to their financial aid officer.

F. Personal Requirement
1. Applicant must complete scholarship application providing an appropriate response to each inquiry.
2. Applicant must complete an essay of 600 words or less (typed) to include a biographical sketch and a description of why the applicant decided to attend PA School and what they foresee themselves doing in their future employment as a Physician Assistant.

G. Non-Discrimination Statement
Eligibility and selection shall not be denied, abridged or affected because of sex, color, creed, race, religion, ethnic origin or physical impairment. Neither will the student director and/or any student participating in the WVAPA be denied, abridged nor affected.
